5. Medical certificate 075-Y, psyx-certificate, drug dispensaries, fluorography, in one PDF file;
6. Vaccination card;
7. Photos 3*4 – 2 pcs., digital photo 3*4 (JPEG);
8. Attached certificate for young men under 27 years of age who have not passed military service (original + 1 copy), in a PDF file;
9. For those who have served: a copy of the military ID card and a certificate confirming the passage
